Add dynamic rows with dynamic values

Hello,

I want to add dynamic rows with form elements in each row.
Basically like this:
{Form.radio button} {Form->checkbox} {Passenger Name} {Image}
{Form.radio button} {Form->checkbox} {Passenger Name} {Image}
{Form.radio button} {Form->checkbox} {Passenger Name} {Image}

This is a dyamic passenger list that need to be generated.

How do i go about doing this using active widgets.?
